APPOINTMENTS
Our appointment schedulers are available
Monday – Thursday from 9:00 AM -4:00 PM and Friday 9:00 AM – 2:30 PM. Their telephone number is 404- 252-3898.
We participate in most of the insurance plans. However, due to the many networks that the insurance plans have we suggest
that you check with your plan to verify that the doctor you are seeing is in their network. Please have your insurance card
available when you call . Plan
to arrive at our office 15 minutes BEFORE your appointment time. This is required in order to complete or update necessary
information in our database. Also please be advised that we have a 20 minute late policy. This policy requires that any patient
arriving 20 minutes past their scheduled appointment time be rescheduled. Should you find that you need to cancel an appointment
we require a 24 hour cancellation notification. Since many of our services are driven by your insurance plan it is imperative that you bring your
insurance card with you at the time of your appointment. If you do not have your insurance card with you it will be necessary
to reschedule your appointment or you will have the option of paying for the services at the time of the visit. Please check
our Insurance Page to see if our health care providers participate on your plan.
FORMS You may download and fill out some of the forms
needed for your office visit from our FORMS page. It would be very helpful and save time if you would bring these completed
forms with you to the office and present them when you check in at our front desk.
PAYMENTS If we are a participant in your insurance
plan than we will gladly file your claim for you. Co-payments and/or deductibles are payable at the time of service. If we
do not participate in your plan than payment in full will be required at the time of service unless prior arrangements have
been made.
PARKING Pay parking is available
in the parking lot immediately behind and adjacent to our building. The fee is a flat $3.
